Advance Health Care Directive (Living Will)
The state of California has a specific approved form to execute an Advance Health Care Directive. This legal document, which is also called a Living Will, permits you to state your healthcare wishes in a way that will permit you to name trusted person to act as your agent to make a broad range of healthcare decisions on your behalf if you are incapacitated and cannot make those decisions on your behalf.
